Are you living with a chronic condition (including but not limited to hypertension, arthritis, chronic pain, COPD, congestive heart failure, diabetes, kidney disease, and/or any other condition that affects one's quality of life)?
Then the Chronic Disease Self-Management Program (CDSMP) might be for you! This interactive workshop is designed to improve an individual's self-management skills for their chronic health conditions. Topics covered in this workshop include nutrition, appropriate exercise, effective communication with family, friends, and healthcare providers, the safe use of medications, pain and fatigue management, relaxation skills, coping with depression, decision-making, and action planning/problem-solving.
The goals for the CDSMP workshop are:
- Improved health literacy
- Patient activation for self-management
- Increased physical activity
- Improvement in: depression, unhealthy physical days, medication compliance
- Better health outcomes: reduction in fatigue, pain, shortness of breath, stress, and sleep problems
- Fewer sick days
- Reduced emergency department visits and hospitalizations
This program consists of six (6) weekly, 2.5 hour sessions
This class is presented in partnership with Oasis.
Through EDUCATION, ADVOCACY, and relevant PROGRAMS, the Oasis impact reaches communities in St. Louis and beyond.
